Insurance coverage and authorization requirements depend on your plan. Some HMO plans require a referral from your primary care physician, and authorization may be necessary before consultation or treatment. Check with your insurer and the treatment center.
Support with coverage
Financial counselors can help you understand coverage and discuss questions about your benefits. Ask about your deductible, copayments, coinsurance and any required authorization before treatment begins.
Hospital and physician bills
